Pour faciliter l\u2019acc\u00e8s aux soins, nous fonctionnons au forfait<\/b>, c\u2019est-\u00e0-dire qu\u2019un contrat est \u00e9tabli entre vous, votre mutuelle et notre maison m\u00e9dicale<\/b>. Tous les mois, votre mutuelle nous versera une somme fixe appel\u00e9e \u00ab forfait \u00bb, couvrant les prestations de m\u00e9decine g\u00e9n\u00e9rale et d\u2019infirmier. Une fois inscrit et en ordre d\u2019assurabilit\u00e9, vous ne paierez plus ces prestations. Attention, ceci est valable uniquement pour les prestations de m\u00e9decine g\u00e9n\u00e9rale et infirmier (y compris les soins \u00e0 domicile) de votre centre.\u00a0 <\/span><\/b>En d\u2019autres termes, vous devez toujours vous rendre au sein de votre centre pour la m\u00e9decine g\u00e9n\u00e9rale et les soins infirmiers.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

\u00c0\u00a0 <\/span>ne venir consulter qu\u2019au sein de notre maison m\u00e9dicale, pour les soins infirmiers et en m\u00e9decine g\u00e9n\u00e9rale. Tout soin chez un autre prestataire dans la zone g\u00e9ographique donn\u00e9e ne sera rembours\u00e9 ni par votre mutuelle ni par nous.<\/b><\/p>

\u00c0\u00a0 <\/span>\u00eatre en ordre d\u2019assurabilit\u00e9 et de paiements de cotisations aupr\u00e8s de votre mutuelle. – R\u00e9sider dans la zone d’inscription reprise sur le plan. <\/b><\/p>

– \u00catre en ordre d\u2019assurabilit\u00e9 aupr\u00e8s de votre mutuelle<\/b><\/p>

– Vous rendre \u00e0 l\u2019accueil, muni de votre carte d\u2019identit\u00e9 et de 2 vignettes par personne, o\u00f9 un membre de notre \u00e9quipe vous expliquera les modalit\u00e9s \u00e0 suivre.<\/b><\/p>

– Toute personne \u00e0 partir de 14 ans doit \u00eatre pr\u00e9sente lors de l\u2019inscription afin de signer son contrat d\u2019inscription.<\/b><\/p>

Vous avez toujours la possibilit\u00e9 de vous d\u00e9sinscrire gratuitement en remplissant un formulaire de d\u00e9sinscription<\/b>.
